Perinatal, Parent & Infant Mental Health – care pathway

What the PIMH Pathway is

A hybrid stepped‑care model integrating digital access, peer support, relational triage, and flexible interventions.

It combines:

  • Online psychoeducation & self‑help
  • Moderated parent communities
  • Digital screening & ABC self‑check tools
  • Relational triage with shared decision‑making
  • Hybrid interventions (online, group‑based, blended, face‑to‑face)
  • Flexible up‑ and down‑scaling of care intensity
  • Monitoring of parental stress, relational functioning and infant signals

The pathway supports parents from orientation to acute care, with continuous attention to the parent–infant relationship.

Why the PIMH Pathway?

Too many families face long waiting lists while relational stress continues to grow. Support is often focused on symptoms rather than relationships.

Not more care.
Better organized care.

What we see so far

Early clinical experience suggests:

  • – Reduced parental stress
  • – Improved engagement
  • – Faster access to support
  • – Prevention of crisis‑driven referrals
  • – Strengthened parent–infant relationships
  • – Higher satisfaction with digital + hybrid care
  • – Better containment in the early phases
  • – More efficient use of specialized care

Families report feeling seen, supported, and less alone.

Why this matters for IMH Worldwide

Hybrid stepped care operationalizes IMH principles by shifting from delayed, symptom‑focused treatment to early relational containment, parental empowerment, and network‑based support.

The model is scalable, translatable, and adaptable to other countries — especially where waiting lists and fragmented care systems hinder early intervention.

Next Steps (2026–2027)

  • – National evaluation with Leiden University (pre‑post design)
  • – Expansion of themes (trauma, bereavement, complex care)
  • – Strengthening collaboration with social domain & regional IMH teams
  • – Standardizing relational triage; exploring automation & Shared Decision Making with parents
  • – Business case development for insurers & municipalities
  • – International translation of digital tools and modules
  • – Scaling hybrid stepped care across PIMH networks
  • – Physical tools as a toolbox for pregnant families distributed by midwife and hospital.
